- Passport biographical page
- Proof of funding for all funding sources
- J-1 Student Other Funding Worksheet, if the student is receiving funding from any source other than Personal Funds or Program Sponsor Funds.
- Proof of English Language Proficiency
- Statement of student’s academic objective (For degree-seeking students, admission letter will suffice.)
- Declaration of non-degree student's choice of TSHIP or GeoBlue insurance (could be a list)
